Patient with Wolff-Parkinson-White Syndrome undergoing Non-Cardiac Surgery. Perioperative Management

چکیده
Background: Perioperative management of patients with preexcitation may become quite challenging, especially if there is no time for adequate preoperative investigation and clinical optimization, as in emergencies, or even worse, undiagnosed cases. It possible that the poorly controlled unknown underlying electrophysiological abnormality will unmasked during anesthesia surgery, giving rise to potentially life-threatening arrhythmias. In literature, pre-excitation syndromes have been mainly approached from view disease (i.e., presentation, diagnosis, treatment), while anesthetic data are scarce. This case report aims focus on perioperative WpW. Patient with Wolff-Parkinson-White Syndrome
Epicardial excitation was explored by means of an exploring electrode during operation on a patient with a large atrial septal defect of the secundum type, whose ECG indicated a Wolff-Parkinson-White syndrome of type B. Very early excitation occurred, 10 msec after the end of the P wave, at the right lateral border, near the atrioventricular sulcus, an area which is located a relatively large d...

Forty-nine patients with Wolff-Parkinson-White syndrome, ages 7 weeks to 51 years, were studied with isopotential body surface maps during normal sinus rhythm, atrial pacing or induced atrial fibrillation. The location of the accessory pathway was determined by multicather electrophysiologic study or surgical ablation of the accessory pathway. When fusion was minimized and ventricular activatio...

This paper reports a patient with Wolff-Parkinson-White syndrome who presented with a tachycardia showing an unusual QRS morphology closely resembling that of a ventricular tachycardia. On reversal to a normal rhythm the electrocardiogram showed changes of a type A preexcitation, with subsequent conversion to a type B pattern. This phenomenon was observed on two separate occasions. Such convers...

متن کامل[Wolff-Parkinson-White Syndrome].
Wolff-Parkinson-White (WPW) syndrome is a cardiac conduction disorder that presents with potentially life-threatening consequences. Wolff-Parkinson-White syndrome-induced dysrhythmias account for 20% of all supraventricular tachycardias that occur in the general population. Clinical presentations range from no symptoms to a sudden cardiac arrest. The risk of sudden death is always present with ...
